The University of Wisconsin-Fond du Lac and Clarity Care are offering a three-credit course this fall to prepare students to take the Certified Nursing Assistant (CNA) exam and earn a CNA designation. For information regarding the schedule visit the Healthcare page.

To register for the Nursing Assitant class it is necessary to complete paperwork BEFORE the start of the class. We have tried to streamline this process for you. Please follow the steps below and contact the Continuing Education office if you have questions: (920) 929-1153 or rhonda.stucky@uwc.edu

Steps to Register for the Nursing Assistant Class

1) Contact the Continuing Education Office

Contact the Continuing Education Director, Rhonda Stucky, at (920) 929-1153 or rhonda.stucky@uwc.edu to let us know that you would like to take the class.

2) Get a TB Test and flu shot

A current TB test is required before the first day of clinicals.

Step A: Print this form:

Step B: Make an appointment to get a flu shot and TB test and take the printed form with you.

Tests may be done through student’s personal doctor, at a local clinic of choice, or at the Fond du Lac Health Department.
Have the provider write a note/print a report confirming that you got a flu shot and fill out the TB test form that you printed above. Bring this written verification to the Continuing Education office or to the first class.

Test cost is the responsibility of the student.

Test MUST BE READ or co-signed by an RN, MD, APNP, or PA-C.

High School Students

Please call the Continuing Education Office at (920) 929-1153 to discuss registration options.

For Credit

If you are taking the class for credit there are two options:

  1. Register as a "Special/nondegree Student" and pay for the class. Apply online. Be sure to choose "Special Student."
  2. Register as a degree-seeking student and apply for Financial Aid. Please contact the Student Affairs office (920) 929-1122 to start this process or visit the Application Process page for more information. (Note: There is no financial aid for Winterim. Contact the Student Affairs office for more information.) Be sure to notify the Student Affairs office that you are enrolling in the course HES 150: Introduction to Patient Care.

Not for Credit

If you are taking the class not for credit, it is not necessary to register as a UW student. Please continue with Step 5 below.

 

5) Call the UW-Fond du Lac Continuing Education office to make an appointment

If you have the above information completed the appointment will take 10 – 15 minutes to review the materials and take a simple reading test.

Bring to the appointment:

  • TB test results
  • Forms listed above (Cover Sheet, Functional Abilities Requirements, Clarity Care Class Payment Refund Policy, Clarity Care Clinical Attendance Policy, and Background Information Disclosure)
  • Social Security card
  • Drivers License
  • Full payment, deposit of $200, or payment confirmation if completed in Step 4 above.
