Student Accessibility Services
Student Accessibility Services are available to eligible students who have a documented condition that requires additional services in order to access the programs and services offered by UW-Fond du Lac.
The Student Accessibility Services Office coordinates services for students with disabilities attending the University of Wisconsin Colleges. The Student Accessibility Services Office works with campus staff and faculty to ensure that students with disabilities who have program access needs receive appropriate accommodations.
In order to initiate the application process, you will need to complete the Student Accessibility Services request form. These forms are available through Student Services. You will also need supporting documentation of your disability from an appropriate professional, and in general less than 3 years old.
t is suggested that you submit the appropriate paperwork several weeks prior to the start of the semester, this will help to facilitate your having support services in place when you need them
